A Product Liability Release form is used when a claim results from the use of a product which creates or causes injury.
The Product Liability Release or proposal form includes name of person signing the release, name of the person being released from liability, the amount paid to obtain the liability release, description of the product which caused the injury and the date the injury occurred, signatures and date of the document.
